Field Trip Forms

Clear instruction and communication with parents and guardians is necessary for taking students on a trip. The following form was given to and returned by students, allowing them to participate in a field trip and performance for the Bayonne Dance Program. Over 130 students from Midtown Community School attended this trip and performed in a show of over 250 students.
